Jeremy serves as a physician assistant for Dr. Dave Weinstein, evaluating and treating orthopedic and sports medicine conditions. Jeremy assists in surgical procedures including arthroscopy and ligament and joint reconstruction. He is skilled in fracture and trauma care of the lower and upper extremities, including knee, foot, ankle, shoulder, and elbow. A Colorado Springs native, Jeremy graduated from Chicago Medical School’s Finch University of Health Sciences. Before receiving his PA certification, Jeremy worked in physical therapy and as an exercise physiologist for many years. He is active in orthopedic research and education.

Ashley Taylor, FNP-BC

Board Certified Nurse Practitioner

Ashley is a board-certified nurse practitioner specializing in orthopedics and sports medicine. She works alongside both Dr. David Weinstein and Dr. Gregg Martyak, evaluating and treating shoulder, elbow, hand, and knee conditions. Ashley spends most of her time caring for patients in the clinic setting but also assists with surgeries, including arthroscopies, fracture repairs, and joint replacements among others. She is a firm believer in patient-centered and evidence-based medicine. Ashley strives to provide her patients with holistic care so they can reach their goals and optimize their health.

Shannon M. Constantinides, PhD, MSN, FNP, NP-C, RN

Nurse Practicioner

Dr. Shannon Constantinides received her PhD from Florida Atlantic University, where she specialized in the use of advanced applied biostatistics, population health, and healthcare delivery systems. As a researcher, Dr. Constantinides studies issues pertaining to musculoskeletal health and social determinants of health. She has authored many peer-reviewed papers and book chapters in the areas of orthopedics and sports medicine, applications of statistics in clinical practice, and emancipatory philosophy. She has also written and regularly gives talks on similar topics.

My meeting with Dr. Weinstein and his PA was wonderful. All questions were answered and explained in great detail. My wait time was rather lengthy as my appointment was at 11 and I wasn’t seen till after noon. I arrived early as requested at 10:30, so I was in the office over 2hours. I would guess more time should be allocated to each appointment to avoid such a lengthy visit!

It's been one year today since I had my shoulder replacement surgery, with the help of the physical therapy of 0CC and many more hours of my home therapy lm happy to say it was a great discussion to have my surgery. I don't have any of the pain I was living with before the surgery, Dr Weinstein also did my rotator cuff surgery over ten years ago and it is still doing great, but bottom line is you have to do the physical therapy religiously to get the best results

Dr. Weinstein took the time to listen and explain my shoulder dislocation in a way that I could relate to and truly understand it. All too often doctors are either too brief or have long, tired responses. Not the issue with Dr. Weinstein. He had the perfect amount of context while providing a clear path forward with a Physical Therapy referral. As a bonus, his staff (XRay chica + Ashley) are professional, knowledgable, and friendly.
